How It Works
The Referral Process
As a specialist clinic, we require a physician referral. This ensures we have your medical history and can prepare for your first visit.
Your Doctor Sends a Referral
Ask your family doctor or another physician to fax a referral to our clinic. The referral should include your relevant medical history, imaging results, and current medications.
We Review & Contact You
Our team reviews every referral promptly. We will call you to schedule your initial consultation at a time that works for you.
Your First Consultation
Dr. Rahimi conducts a comprehensive assessment, reviews your history, performs a physical exam, and discusses treatment options tailored to your condition.
Personalized Treatment Plan
Together, we develop a treatment plan that addresses your specific pain condition, goals, and preferences. Treatment may begin the same day in many cases.
Be Prepared
What to Bring
To make the most of your first visit, please bring the following items.
Ontario Health Card
Your valid OHIP card for covered services.
Photo ID
A government-issued photo ID to verify your identity.
Medication List
A complete list of all current medications, dosages, and supplements.
Imaging Results
Any X-rays, MRIs, CT scans, or other imaging on CD or USB.
Previous Medical Records
Reports from other specialists, surgical notes, or relevant test results.
Insurance Information
Extended health benefit or workplace insurance details for non-OHIP services.
Your First Visit
What to Expect
Your initial consultation typically lasts 30-45 minutes. Dr. Rahimi will take the time to thoroughly understand your pain condition, its history, and how it affects your daily life.
The visit includes a comprehensive physical examination, review of all imaging and test results, and a detailed discussion about diagnosis and treatment options. We encourage you to ask questions and share your concerns openly.
In many cases, we can begin treatment on the same day as your consultation. For procedures that require scheduling, our team will arrange the earliest available appointment.
After your visit, we send a detailed report to your referring physician to keep them informed about your care plan and progress.
